Select which info option you want to display. With Contact Info will show a card on the right with contact information set in Site Options. Without Contact Info will not show the card.
The ID dropdown lets you choose which form you want to use.
Use the WYSIWYG Editor to add the content that will appear above the form.
If With Contact Info is selected there will be an input field to add the title for the contact card. This will appear above the phone number.
Add your background image that will appear on the right. On mobile With Contact Info will switch the order with the card on top and form on bottom. Mobile for Without Info will remove the image and just show the form.
